Please note that functional services are not covered by insurance and are considered self-pay. Payment will be the responsibility of the client at the time of service.
INSURANCE/BENEFITS
Cancellations must be made at least 24 hours in advance. Appointments canceled with less than 24 hours’ notice will incur a $50 fee.
